The Children's Case Management Program assists
families in identifying and meeting a wide range of needs for their child
with a mental health diagnosis or with Mental Retardation. Program Description Case managers work with
families to develop individualized service plans to identify and meet needs
in areas including therapy, psychiatry, behavioral services, educational
advocacy and consultation, social/recreation, coordination of treatment teams
and plans, accessing benefits, and support and guidance. Target population is children
age birth through 20 who have a mental health diagnosis or mental retardation
and reside in Kennebec or Program is staffed by experienced case managers with a minimum of LSW licensure, supervised by master's level staff. Case managers meet with
families in home, school, office, and community settings on a regular basis
to develop service plans and meet goals. Service continues until goals are
